As you may already know, he didn't leave the band because he wanted. It was Mana who kicked him out (or so I've heard). Anyway, I think I know how it happened. Mana called for a meeting only with the other 4 band members and called a vote to kick Gackt out of the band. OFC Gackt refused so the score was 1 (for kicking Gackt out of da band) -1 (for lettin him stay in da band). Yu~ki voted to kick Gackt out because as you know the least amount of photos in which only two members of Malice Mizer appeared togheter and in which one of em was Gackt was Gackt and Yu~ki. And Yu~ki motivated that Gackt didn't even give a shit about him. Most of times he didn't even say hi to him when the band members met (for making music or whatever other reason). Kami voted Gackt to stay since they were very close friends. And then Mana told Közi: It's your turn to decide, will Gackt stay in or will he get out? And Közi was like: You know Gackt, I like you, you are a great vocalist but I'm afraid I will have to go with Mana on this one since I've known him for a longer time than I've known you and besides he's the leader. And Gackt was like: After all I've done for you, FUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUUU.

I heard it's because a fan sent them a waffle maker. Only one. And both Mana and Gackt really wanted it. So Gackt said "if u do not give me waffle maker I leave".
And Mana replied, "i chose waffle maker gakuro."
Then Gackt was sad because Mana did not remember his name and liked waffle makers more so he left
Nah, but speaking of silly whatever. Some girl I met insisted that Gackt told her it was a love rivalry over Kami XD She also said they used to throw stuff at each other like shoes and microphones lol

Gackt seems much more careful with choosing his friends. He wouldn't befriend someone who would sell him out for a latte...

Well, according to Gackt's autobiography, yes, he was kicked out. Actually I think this is the most detailed thing I've seen about Gackt leaving Malice Mizer, from the words of himself.

Didn't it say something like, they had this meeting without Gackt, and came to the decision that they were going to break up. They called in Gackt afterward, and then Gackt implied in his book that either Kozi or Yu~Ki said "Lol think of all the money we made off these sucker fans." To which Gackt replied, "No, don't say that. How can you treat fans like this? You are so important to them, why aren't they important to you? I will leave the band so you four can stay together and keep the fans happy."
I would end this with a joke, but I think Gackt already did the job for me.

I dunno, I read the autobiography and it could be that's really what Gackt believes happened from his point of view. Mana may have had a completely different point of view, as did Kozi, Yu~ki, Kami, the president... But as far as I know, they haven't openly talked about it. I guess to get the real truth, you'd have to have a completely unbiased person who was actually there and saw what went down, but I doubt you'd find such a person... Gackt was probably being completely honest when he wrote what he wrote. (But don't most interviews imply that it was actually Kami who kept the band from breaking up?
